How to Make a Referral
A referral for hospice care may be made by anyone—including the client.
Referrals from individuals are accepted by telephone to
416-385-8885 ext. 1.
All referrals from health care professionals require completion of the Palliative Care Common Referral Form available here. Please fax the completed form to 416-385-8887.
Admission Criteria
In-Home Hospice Care
The individual must live within the geographic service area of the hospice, or be a support person to an individual living with a life-threatening illness who resides within the service area. They must also consent to receive services. Anyone may make a referral to the program.
Day Hospice Program
The Adult Day program is open to people who are palliative. Participants must be independent in functions of daily living and be able to take their own medications. An assessment by telephone is required. Anyone may make a referral to the program.
Support & Bereavement Services
Support and Bereavement Programs are available to individuals living with a life-threatening illness, their caregivers, and anyone who has experienced a loss and is finding it difficult to cope, whether or not the deceased was a client of the Hospice. The program is open to anyone regardless of where they live but in-home bereavement support is available only to people living within the geographic service area of the hospice (Lake Ontario to Steeles Avenue from the Humber River to Victoria Park Avenue). Anyone may make a referral.
